Share Transfer (US Shares)	US Shares Transfer Form
Issuer Sponsored Transfer (AU Shares)	Issuer Sponsored Transfer Form	Government issued photo ID (driver’s licence or passport). It must show your signature Holdings statement (or dividend statement) dated within 12 months, showing the full SRN (not redacted).

Please note: 

• For transfers from Australian Investment Exchange (AUSIEX), please supply a certified copy of your photo ID
• For Broker to Broker transfers, these can be completed paperless via the Superhero web platform. These forms are only necessary if you can't access the Superhero web portal.
